Instructions for Completing this Form

Before you begin this application, make certain that you have the following items available as you will need to upload them to this application.

  1. Completed and signed scanned copy of the Mentor Approval form Mentor Approval form
  2. Your Abstract

NOTE: Group Projects: Select ONE group member to submit ONE application on behalf of the group. Upload completed and signed Mentor Approval forms for each group member to this application.
EX: If there are a TOTAL of 4 students in your group then you must upload 4 Mentor Approval forms to this application.

****NOTE: Art Students must submit the Arts Supplementary Application in addition to this application.
